Tier II Program Reporting Requirements
CVS Health requires suppliers who participate in the Tier 2 Program to submit reporting detailing diverse vendor spend associated or allocated to any goods or services provided to CVS Health.
The following supplier classifications are accepted for Prime Suppliers and General Contractors:

Note: CVS Health does not accept General Contractor spend with Small Business Enterprises (SBEs) as part of diversity reporting
Reports must be submitted quarterly on the following dates:
- Quarter 1 (January 1 - March 31):
- Primes: Period opens April 1st | Reports due by May 15th
- General Contractors: Period opens May 8th | Reports due by June 6th
- Quarter 2 (April 1 - June 30):
- Primes: Period opens July 1st | Reports due by August 14th
- General Contractors: Period opens August 8th | Reports due by September 6th
- Quarter 3 (July 1 - September 30):
- Primes: Period opens October 1st | Reports due by November 14th
- General Contractors: Period opens November 8th | Reports due by December 7th
- Quarter 4 (October 1 - December 31):
- Primes: Period opens January 1st | Reports due by February 14th
- General Contractors: Period opens February 8th | Reports due by March 9th
